HealthSnap: Continuous Remote Patient Monitoring as a Scalable Intervention

This study by HealthSnap examined patients with uncontrolled high blood pressure who had been enrolled in RPM programs and transmitted data for at least 16 out of a 30-day period. Patients received education and support from licensed clinical staff, who reviewed readings, engaged patients in real time conversations, and reinforced care plan adherence. Patients with elevated baseline fasting blood glucose were also examined.

Systolic blood pressure improvements were identified as early as <100 days of RPM usage, with most substantial reductions identified in patients engaged with RPM for 450 days or more. The consistency in effect size beyond this point underscores the durable benefits of long-term engagement. Similar trends were seen regarding diastolic blood pressure. Outcomes align with high clinical relevance and potential cardiovascular risk reduction.

Meaningful glucose reductions were observed as early as <150 days, with more sustained and substantial improvements observed with longer participation of 600+ days.
